simple, buy phone gateway, such as avaya ip office 400

then get a pri-t1, so you can route 23 phone channels/lines to your eqipment

buy a pool of say 100 phone numbers that come in over that pri line

get an internet T1 so you can serve you calls out to the internet

put voip phones in clients place, make sure they have internet connection
